Scope

This information provides guidance on the management of Healthcare for Older People (HfOP) patients across Eastern Devon during the COVID-19 pandemic. The information is based on national (British Geriatric Society) guidance.

Conditions with specific guidance

Parkinson's
  • Community Parkinson's Disease nurses are on 01392 405477
    • cover Exeter, Okehampton and villages and Crediton GP's
  • for mid and east Devon (not Crediton GP's) it's 01884 235552
  • If that fails, then hospital-based Parkinson;s Disease nurses can give some advice on 07824 483324.
Falls
  • Please contact the Acute Care of Elderly (ACE) Geriatrician line 07702 528762

  • For urgent advice regarding frail older people, regarding admission avoidance or admission (COVID or non-COVID)
Stroke
  • Urgent stroke clinic referrals: rde-tr.dailystrokeclinic@nhs.net
  • Can call the stroke team administrator for a stroke clinic referral on 01392 402552 if the patient is with the referring clinician
  • All patients with possible TIA are telephone triaged and some are offered face-to-face review
  • Stroke Nurse Practitioner / Front-line stroke consultant or neurologist: tel: 07825 716447

Community Support Line:

Advice line to support GPs, community teams, to include Devon Partnership Trust (DPT) colleagues, and care home staff in the care of adults, with or without multiple co-morbidities, long term conditions and frailty, regardless of COVID status

  • 01392 411611 and request 'Community Support Line'
  • staffed by a consultant of a general medical specialty 9am - 5pm Monday - Friday
    • Aim is to support pragmatic decision making and to avoid unnecessary admission to hospital
  • Requests for admission continue through the RD&E Admissions Coordinator, as usual
